Your journey begins with either a video or phone consultation with Dr. Rapaport.  

Dr. Rapaport understands your need for close and prompt communication as well as the importance of support between appointments. Because she is the only physician at Haven Medical, she limits the number of patients she sees so that she can provide the kind of care you need. 

In order to be considered for care at Haven Medical, we ask that you contact us for a New Patient Inquiry form. Dr. Rapaport will personally review your information and if she believes she can help, you will be offered an initial video or phone consultation. During the consultation Dr. Rapaport will talk with you to develop a mutual understanding of your current symptoms and priorities along with an outline of possible causes of those symptoms and potential treatment ideas. When you move forward with care, the charge for the consultation will be credited toward your recommended evaluation or treatment program.
